Multiple Sclerosis Treatment in Redlands

Some mornings your legs cooperate. Some mornings they do not, and you do not find out which kind of morning it is until you stand up. MS changes the signal your nerves can carry. It does not get to decide how strong, how steady or how capable the body receiving that signal becomes.

01What Causes It

What Causes Multiple Sclerosis Symptoms?

You have been told to rest when you flare and told to exercise so you stay strong, and nobody has explained how those two instructions fit together. Meanwhile the leg that dragged a little last spring drags a little more now. Here is what is going on underneath. Multiple sclerosis damages the myelin that insulates nerve fibers in the brain and spinal cord, so signals arrive late, faint or out of order. The muscle receiving that signal is usually still healthy tissue. Some of the walking, balance and endurance you have lost is disuse sitting on top of the neurological change, and disuse answers to training.

MS damages the myelin sheath that insulates nerve fibers in the brain and spinal cord. With that insulation stripped, the message from brain to muscle arrives late, faint, or out of step with the message going to the muscle beside it. That timing failure is why a movement feels heavy and clumsy even when the muscle itself tests reasonably strong.

Part of what your brain normally sends down the spinal cord is a signal to relax. Interrupt it and muscles hold on instead of letting go: a stiff calf, a hip that will not swing through, a leg that goes rigid when you are tired. Spasticity makes walking cost more energy than it should and pulls joints toward positions they get stuck in.

A small rise in core temperature slows conduction along nerve fibers that are already damaged, so symptoms look worse in the heat even though nothing new has happened to the nerve. In a Redlands summer that matters. It is why a hot afternoon or a hot shower can leave your vision blurry or your leg dragging for an hour afterward.

Balance runs on three inputs: what your eyes see, what your inner ear senses, and what your joints and feet report about where they are in space. MS commonly degrades that third input, so your body loses track of your own limbs, especially in the dark. Most people compensate by staring at the floor and widening their stance, which works right up until something unexpected happens.

MS fatigue is not ordinary tiredness and it is not a motivation problem. Part of it is neurological and part of it is mechanical: an inefficient walking pattern burns far more energy per step than a clean one, so the reserve is gone by early afternoon. Making the movement itself cheaper gives some of that reserve back.

After a bad relapse most people pull back from anything that felt risky, and the pullback outlasts the relapse by months. Muscle mass, cardiovascular capacity and confidence fall together, and that fall produces weakness that gets read as further progression. Supervised, appropriately dosed strength work is well established as safe in MS.

02How PT Helps

How Physical Therapy in Redlands Helps

Physical therapy does not change the disease. What it changes is everything the disease has to work against: how much strength you hold in reserve, how efficiently you walk, how well you catch yourself when you stumble, and how much of your day is still yours. A doctor of physical therapy measures where you are now across gait, balance, tone, strength and endurance, then builds a plan around your fatigue instead of in spite of it. Sessions are one on one and run 30 to 60 minutes.

03What To Expect

What to Expect When You Start

01

The evaluation

Your first visit runs about an hour, one on one with a doctor of physical therapy. You will talk through your diagnosis, your relapse history and what a genuinely bad day looks like, because a plan built for your best day is useless. Then the exam: walking speed and pattern, balance under several conditions, tone, strength, sensation and endurance. In California you can begin physical therapy without a physician referral for up to 45 days or 12 visits, whichever comes first, and we verify your insurance benefits before you commit to anything.

02

Your plan, in four phases

Every plan here moves through Pain, Prime, Perform, Prevent. Pain calms whatever is limiting you enough that you can move and be assessed properly. Prime restores the range and control that have quietly gone missing. Perform rebuilds capacity for the specific things you want back, whether that is standing through a service, carrying groceries in from the car, or walking the dog around the block. Prevent locks the gains in with a home program sized to the energy you actually have.

03

Measured, not guessed at

Most plans of care run 2 to 3 visits a week for 4 to 6 weeks, and many patients notice a change in the first week or two. With MS the target is not a finish line. It is holding function, catching a decline early, and knowing which parts of a rough month are the disease and which parts are fixable. Your therapist re-tests your measurements at every visit, so progress is measured rather than guessed at.
